Department of Human Services Cranston Field Office to Close on Monday, November 3, 2008

The Department of Human Services Cranston Field Office located in the Aimee Forand Building on 600 New London Avenue in Cranston will close at the end of the day on Monday, November 3, 2008. Individuals currently served at this office will be reassigned to existing field offices. This move will make way for renovations to the Aimee Forand Building.

The move impacts individuals who live in Cranston, Johnston, Scituate and West Warwick receiving or applying for Medical Assistance, Food Stamps, and Rhode Island Works cash assistance (formerly know as Family Independence Program or FIP).

All State Offices are closed on Tuesday. Beginning Wednesday, November 5, individuals from these communities should seek services at the following offices:

Field Office to Use as of November 5, 2008:

Cranston, Johnston, Scituate - Providence Regional Family Center, 206 Elmwood Avenue, Providence, RI 02907, Telephone: 222-7000

West Warwick - Warwick Field Office, 195 Buttonwoods Avenue, Warwick, RI 02886, Telephone: 736-6511

Individuals who live in these cities and towns who are only applying for Food Stamps and not any other benefits (Medicaid or Rhode Island Works Cash Assistance) should use the Warwick Field Office located on 195 Buttonwoods Avenue in Warwick.

This move does not impact those individuals applying for Long Term Care Services. Individuals living in Cranston, Johnston, Scituate, and West Warwick applying for Long Term Care Services will continue to seek services at the Benjamin Rush Building (Building 55), located on Howard Avenue on the Pastore Campus in Cranston.

Applications for Child Care Assistance will continue to be accepted via mail and should be sent to the Providence Regional Family Center, 206 Elmwood Avenue, Providence, RI 02907.

The Department of Human Services sent a letter to program beneficiaries notifying them of the changes in service location.
