The attached survey is a follow-up to your recent environmental permit application with the RI Department of Environmental Management (DEM.) We are interested in improving our permitting system, and to do so, need your honest input. Your participation in this survey is voluntary and can be done anonymously. Your feedback will help us improve the quality of our permitting programs, so we encourage you to take a few moments to complete this survey.
This survey should only apply to the experience that you had with the Office of Technical & Customer Assistance. If your experience concerning any other program is different from the customer service provided by the Office of Technical and Customer Assistence, please fill out another survey that is located on this website.
