For each licensee, the following fields are contained within the data file:

• Zip Code
• Phone Number
• Email Address
• License Type 1
• License Number 1
• License Status 1 (Curr)
• License Type 2
• License Number 2
• License Status 2 (Curr)
• License Type 3
• License Number 3
• License Status 3 (Curr)
• License Type 4
• License Number 4
• License Status 4 (Curr)

Each Licensee may have up to 4 licenses, if the licensee has less than 4 licenses, those fields will be blank.
